    Key to understanding CVD (Chronic Venous Disease) is first taking an inside view into how the venous system works. The circulatory system consists of an intricate network of blood vessels that transport blood throughout the body. The heart pumps oxygenated blood into the arterial system, while the venous system brings desoxygenated blood back to the heart. Chronic venous disease is directly related to the health of the peripheral venous system in the legs, which consists of the deep veins (deep within the muscles), the superficial veins (close to the skin) and the perforating veins (connect the deep and superficial veins).

    Calf muscles and venous valves

    During movement and exercise, the calf muscles continually contract and relax. As they do this they squeeze the veins and pump blood through a series of one-way valves upward towards the heart. These valves open when the calf muscle contracts, and then close when it relaxes. This ensures that the deoxygenated blood only flows in one direction, without any pooling or back flow into the veins.

    Healthy veins have a series of valves that open and close to direct blood flow back to the heart. CVD is caused when these valves fail to work properly: Blood can flow backwards through the veins and pool in the legs, increasing pressure in the veins. This can result in severe symptoms such as leg heaviness, aching, dilated or unsightly veins, to severe such as swelling, skin colouration and recurrent venous ulcerations. Compression stockings assist insufficient venous valves by compressing the limb. This reduces the diameter of distended veins bringing the valve cusps close together and increases venous blood flow towards the heart.

    CEAP C0 - Out of sight but developing

    C0 is easily missed, because there are no visual or palpable signs of venous disease. You might complain of aching, tired and heavy legs. At this stage, preventative care can have an excellent outcome.

    CEAP C1 - Signs are beginning to appear

    As the venous system begins to experience valve failure and difficulty with blood circulation, the first visual indicator might be "spider veins". Spider veins are often seen as a merely cosmetic issue and not as a health problem, but it is now time to take action to slow down disease progression. As with stage C0, you may also complain of tired, heavy legs and could experience some minor swelling, which resolves with leg elevation or rest.

    CEAP C2 – Time to get serious

    At C2, the most significant sign of CVD are varicose veins. Venous valve failure is serious enough that back flow and blood pooling are causing vein walls to stretch beyond repair. At this stage it is increasingly difficult for the venous system to pump the blood back to the heart. Other factors could also be contributing to this difficulty, such as lack of exercise or mobility issues, resulting in weakened calf muscles, which are responsible for pumping the blood through the venous system. Even simple exercise, like walking daily, can help keep the calf muscles strong. Varicose veins are unsightly and often painful. Feelings of tired, heavy and aching legs are common. Itching, burning sensation and minor swelling could also be present.

    CEAP C3 – Increasing patient discomfort

    At C3, the primary symptom is oedema. Caused by accumulation of fluid in lower extremity tissues, the oedema is often accompanied by a heavy ache. At this stage, therapy is critical to counteract the progression to more severe symptoms from skin changes to ulcerations.

    CEAP C4 – Serious risk of progression

    At C4 skin changes are present. Skin changes include discolouration which most often appear on the inner part of the ankle or leg. Also the skin can thicken and harden. Once this is present, the skin is damaged and the therapy goal is to prevent further progression to venous leg ulceration and complications such as infection. Because the skin is now quite fragile, extra care must be taken to not cause trauma through friction, for example scratching the skin.

    CEAP C5 – The road to venous leg ulcer (VLU) recovery

    Though C5 is evidenced by healed ulcers as other serious CVD conditions might still exist, the venous leg ulcer can recur and be "active" again (C6). Therefore keeping the skin healthy as part of an overall ulcer prevention plan is key.
